Cétuximab dans le traitement du cancer colorectal métastatique réfractaire à l'irinotécan: évaluation de facteurs cliniques prédictifs de réponse

Abstract : Background : The aim was to identify baseline clinical predictive factors of response to cetuximab-irinotecan (CI) in patients with irinotecan-refractory metastatic colorectal cancer. Patients and methods : Retrospective analysis of patients treated between June 2004 and February 2007 with cetuximab (400 mg/m2 week 1 and 250 mg/m subsequent weeks) plus irinotecan (180 mg/m2/2 weeks). Predictive factors of response to cetuximab were identified using univariate and multivariate analyses according to the following baseline available features: demographic data, disease duration, response to prior chemotherapies, irinotecanfree interval, metastatic sites number, and tumoural immunohistochemical EGFR status. Results : A total of 311 patients were analyzed. The objective response rate under CI was 26 %. ln univariate analysis, a prior response to irinotecan, a disease duration >= 24 months, a metastatic disease duration >=18 months, an irinotecan-free interval >= 1.8 months, and no more than one metastatic site were significantly predictive of response to CI. ln multivariate analysis, an independent predictive value was confirmed for prior response to irinotecan (p < 10-3, RR 1.89), disease duration (p = 0.01, RR 1.75), and number ofmetastatic sites (p < 10-2, RR 1.64). EGFR status had no predictive value. Among 35 EGFR-negative patients, 14 (40 %) achieved'an objective response.Conclusion : Prior response to irinotecan and number of metastatic sites are independent predictive factors of response to CI. This study brings further evidence for also treating with cetuximab EGFR-negative patients.
