Evaluation de la prescription de l'imagerie dans la prise en charge de la douleur abdominale aiguë non traumatique: étude rétrospective de 1019 patients admis au Service d'accueil des urgences du CHU de Nancy

Abstract : This retrospective, descriptive study, carried out in the emergency department of the CHU, Nancy, during one year, collected 1,019 patients'files, presenting an acute, non traumatic, abdominal pain. It demonstrates that if the interrogation, the clinical examination and additional examinations being carried out -biological, plain abdominal radiography- are necessary, they however remain insufficient .A plain abdominal radiography is still asked for in 67.9% of the cases, while it doesn't bring any information in 86.4% of the cases. ln 33.2% of the cases, there is no presumed diagnosis - non specific abdominal pain - after the clinical examination and additional examinations have been carried out. Imagings (ultra sonogrphy and for CT Scan) are thus essential to direct the patient towards a service of medicine, surgery or his/her passage in case of emergency in the surgical unit. Indeed, the rate of non specific abdominal pain is divided by 3 in the reserved diagnosis, after the additional examinations of imaging have been carried out. The presumed diagnosis is confirmed by the additional examinations of imaging in 58.8% of the cases and modified in 41.2% of the cases. Finally, using an ultrasonography and/or CT Scan highlights a link between the diagnosis made when the patient leaves the emergency department and the diagnosis made when the patient is discharged from the hospital.
