Evaluation de la rigidité artérielle chez les patients présentant des anticorps antiphospholipides

Abstract : We studied pulsewave velocity (PWV) and the Pulse Pressure Amplification (PPA) with the Pulsepen system® in 53 patients with APL (AntiphosPholpid antibodies), aged 46±2.3 years, with a majority of women (n=40, 75 %). The patients were asymptomatic (n=11,21%) or syrnptomatic with a primary APS (AntiphosPholipidSyndrome) (n=26,49%) or secondary APS (n=16,30 %) toan auto-immune disease. Among the patients, 41% presented with hypertension, 30 % with a dyslipidémia and 22% did smoke. A control group of 109 healthy subjects was selected. There was a significant increase of the PWV (8,14±0,4 m/s vs 7,24±0,2 m/s ; p=0,036) and periphcral vascularresistances in patients with APL compared with the healthy subjects. Among the patients presenting an APS, the PWV was increased in those having presented an arterial thrombosis (10+- m/s vs 7,5+-O,4 m/s p=O,02). The PPA was significantly decreased in patients with El primary ApS comparedto the patients with asymptomatic APL vVe didn't find a significant difference of the PWV according to the type of APL The inflammatory parameters do not influence directly the PWV. There was no significant difference of the PWV between the patients presenting asymptornatic APL,primary or secondary ApS. The increase of the artcrial stiffness is associated with classical cardio-vascular risk factors. The study of the arterial stiffness could identify a higher cardio-vascular risk profile in patients with APL.
