Devenir à long terme des patients traités pour un lymphome de Hodgkin stades I et II sous-diaphragmatiques, entre 1985 et 1995 à Nancy

Abstract : Methods and materials : 200 patients, aged at least 15 years, received radiotherapy (RT), and 172 of them associated chemotherapy for supradiaphragmatic stages I/II Hodgkin's lymphomas (HL) in Nancy between 1985 and 1995. We first studied the overall (OS), specific (SS) and disease-free survivais (DFS), and then, we recorded ail the late events after the end of treatment. For each group, we looked for a relationship between their occurrence and the treatment, in order to identify a toxicity ofit (statistical test of Log-Rank).Results : The OS, SS and DFS were respectively 99%, 99.5% and 95% at 1 year, 94.5%, 95.5% and 87% at 5 years and 86%, 91 %, 81 % at 10 years. We recorded 3 malignant blood diseases, 8 cancers in irradiated territory, 39 CV events including 4 myocardial infarction and 8 heart failure, 62 thyroid disorders, 33 pulmonary events, 54 cutaneous/mucous complications, and 64 other complications including 17 herpes zosters, 7 Lhermit syndromes, 19 gonadal disorders, and 12 symptoms suggestive of neurotoxicity. We remarked a significant relationship between increased dose of mediastinal RT and the number of CV events in 10 years, RR=2 [1.0-4.1] (p = 0.05).Conclusions : Thanks to follow-up, first we obtained good survivals, comparable to literature, and then a relative toxicity of RT alone, or associated to chemotherapy, which is acceptable. A further study is required to obtain significant data.
