Prise en charge des patients diabétiques suivis en médecine générale en Guadeloupe: Etude transversale à partir du bilan annuel de retentissement du diabète de 178 patients

Abstract : Objectives: the aim of the study was to describe the characteristics and medical care of diabetic patients followed by their general practitioners (GPs) in Guadeloupe, in the light of official recommandations for monitoring and medical care of diabetics patients. Methods : this cross-sectional study by questionnaire on the annual impact of diabetes was conducted among a representative sample of 117 GPs in Guadeloupe, during the November 2006 to November 2007 period. Results : the response rate was 40.2 %. 47 practitionners filled the questionnaire for 178 patients. Among the patients included, 63.5% were women. Among patients, 51.7% were 65 years old or older with diabetes evolving for over 10 years long for 41.0% of them. 66.3% were treated with oral antibiabetic treatment only. Abdominal obesity was significantly higher among women (75.9% of them). 81% of patients were hypertensive. 43.3% had at least 3 HbA 1 c. Lipids, creatinine, and microalbuminuria were filled respectively for 71.3%, 81.5% and 60.7 % of patients. Electrocardiogram and ocular fundus were reported for 65.7 % and 59.0% of them. 15.7 % reached an HbA 1 c target of less than 6.5% and 30.7% of less than 7%. 12.9% had a blood pressure below 130/80 mm Hg. 31.2% had LDL-cholesterol greater than or esqual to 1.4 g/l. 49.5% reached their LDL-cholesterol target. Among those with high cardiovascular risk, 54.7 % were receiving treatment with statins and 61.6% with antiplatelet agents. Diabetes complications were frequent, especially among men and from the diagnosis. Conclusion : GPs in Guadeloupe are highly sensitive to diabetes medical care. Actions could include enhanced therapeutic education, earliest detection of diabetes and earliest intensification of therapeutic strategy.
