Développement d'un outil d'aide à l'interprétation des résultats de tests de Single Antigen HLA dans le cadre de la greffe d'organes

Résumé

The determination of the HLA compatibility at the structural level has proven its usefulness in donor/recipient matching and graft outcomes domains. The release of the HLAMatchmaker computer program has been one of the most substantial contributions in this field. HLAMatchmaker program allows one to obtain an exhaustive list of potential reactive eplets through a two-step approach: (i) emitting beads, referred to as positive beads, are identified using MFI (Mean Fluorescent Intensity) value thresholding and (ii) eplets which belong to all positive beads and do not belong to negative beads are reported as reactive. Such a technique has shown to perform well and extensive validations are reported in the literature but the results interpretations are time consuming because of their presentation even more when a lot of positivity are found in profiles. This paper consists in the presentation of a novel non negative least squares based method to facilitate the identification of the most probably HLA eplets involved in the positive results of single antigen class I tests. Such a model allows one to express the list of potential reactive eplets as a non negative least squares solution making use of all the MFI measurements simultaneously in a single step approach. A validation of the model has been conducted over three patients data. The obtained non negative least squares solutions delivers coherents reactives eplets with the one obtained using HLAMatchmaker (Version connected to the Fusion V4.2). The obtained results therefore suggest that our analysis method may complement the HLAMatchmaker program in order to make eplets analysis easier in a less time consuming and more pertinent/efficient way.
