Evaluation in vivo de l'efficacité d'oxygénation tissulaire d'une émulsion de perfluorocarbure de nouvelle génération en situation de choc hémorragique

Abstract : The objective of this work is to study the capacity of a new generation of a PFC emulsion in restoring tissue oxygenation in tissues that are sensitive to ischaemia such as intestine, liver and skeletal muscle. We first studied the impact of this PFC emulsion on the heological behavior of human blood in condition of hemodilution with various volume expenders. We showed that the rheological behavior of the plasma substitute-PFC mixtures depended both on the type of plasma substitute and the concentration of the PFC emulsion. From this study, we determined the mixture more suitable for use in in vivo and in ex vivo protocols which we had developed. The protocols of severe hemorrhagic shock in rabbit enabled us to show that the resuscitation with a PFC emulsion added with a plasma substitute can increase the level of oxygenation in tissue (measured by specifie microelectrodes) and that this substantial O[subscript 2] delivery in ischaemic tissues did not have any side effect in cellular metabolism (evaluated by microdialysis). We also showed that the PFC emulsion improved the tissue oxygenation by generating a vasodilatation in the microcirculation of the tissue (evaluated by microdialysis with ethanol) and permited to restore rapidly the microcirculatory perfusion and thus peripheral oxygenation. The ex vivo protocol of perfused bowel, developed in the rat, showed that the PFC emulsion allowed the delivery of a sufficient quantity of O[subscirpt 2] to bowel tissue, thus maintaining the peristalsis and the structural integrity of the bowel.
