De l'accréditation à l'intelligence organisationnelle en milieu hospitalier : essai d'une approche méthodologique sur les organisations complexes

Abstract : Total quality in health organizations defines strategie medical objectives, general aims for paramedical care, social policy and information systems management. To this end, a reference framework for accrediting French hospitals was created in 1996. The idea for a program to promote quality in hospitals is almost a century old. In 1917 a American surgeon working on efficiency in hospital care declared: "hospitals should compare their results with those of other hospitals, ... , no only their successes but also their mistakes" / E.A. Codman [Massachusetts General Hospital]. The research undertaken in this thesis stems from Master's degree dissertations, the authors of which mostly have a medical or paramedical background. A study of these documents revealed a convergence of ideas. They are all specific to the subject under study and are great interest to the particular field of study. Nevertheless, a comprehensive analysis of all the dissertations shows there are global strategies for optimising the management of the accreditation process. This research was carried out from an engineering stand point and therefore brings a different vision of the health sector. The world of medicine is different to the world of industry. Without being rationalist with reductive beliefs, quality in health must equate with the rational, which dissociates facts and ideas. Thus is becomes plausible to evoke a methodological approach to developing quality in health. Accreditation is no longer an examination to be passed but sparks progress towards managing change. To be credible, the following aspects must therefore be taken into consideration: placing employees at the centre of all actions, collaboration within teams and quantification of data. Using accreditation as a tool for development, this study aims to present health organizations with another viewpoint on implementing total quality.
