Place du Core Stability dans les interactions posture-mouvement chez le sportif : de la régulation de l’équilibre au risque de blessure

Abstract : During posture-movement interactions, Core Stability will play a central role ensuring balance stability and efficient movement production. Core Stability is defined as the ability to control the trunk with respect to the pelvis through core muscles, to allow optimal transfer or production of force to the limbs. The main objective of this thesis is to study the place of the Core Stability during a continuum of postural tasks, from unstable bipodal stance to dynamic movements, with athletes. Orthostatic postural control is mainly modeled as a double inverted pendulum, around the ankle and hip joint. However, this model does not take the trunk into account, which could play a role in postural control due to its inertia and its central position. Therefore, the first aim of this thesis is to determine the contribution of the trunk in postural control, and its neuromuscular mechanisms during a challenged balance task. We found that the ankle is the main joint controlling the posture, followed by the trunk, the hip and lastly the knee. Trunk motion seems to be mainly controlled by the muscles erector spinae muscles, even if abdominals might have a stabilizing role for the spine. When posture undergo external perturbations, strategies involving core kinematics and neuromuscular control is still unclear. Focusing on neuromuscular co-contractions ratio could provide a better understanding of core kinematic. The second aim is to define the motor strategies and neuromuscular control mechanisms of the Core Stability to maintain balance. We highlighted that the core kinematics was the result of an active strategy and that the use of neuromuscular co-contractions ratios helps understanding its motion.During dynamic sports movements, such as unanticipated changes of direction, expertise can have an impact on both anterior cruciate ligament (ACL) tear risk and Core Stability. Also, some trunk kinematics are known to increase ACL injury risk, but the detailed role of the Core Stability still needs to be deeper investigated. Thus, the third aim of this manuscript is to determine the impact of the task expertise and of Core Stability on ACL injury risk, and also to investigate neuromuscular control of the Core Stability. We found that experts had higher knee joint loading without having an increase of ACL injury risk. Moreover, the three-dimensional kinematics of the Core Stability impacts injury risk, that can be explained by co-contractions ratio between agonists and antagonists’ muscles.
