Modes d'entrée dans une psychose en milieu militaire : indices d'alertes pour le médecin généraliste d'unité. Etude rétrospective à partir de trente dossiers cliniques

Abstract : The beginning chronic psychosis is characterized the more often by an insidious beginning with a sudden relapse in the onset adulthood. This risky period correspond to the moment of the commitment of the young servicemen in the army. The military general practitioner plays here a central role in the early detection of these disorders. Our task was, thanks to a clinical review about this subject, to study 30 young servicemen?s psychiatric files who have made a psychotic outcome as well as the general practitioners's letters who have sent them in order to highlight the warning clues of a higher level risk of a chronic psychosis relapse. The psychotic phase is preceded by a prodromic phase which can last many years and which is characterized by behavior disorders and nevrotic signs which are the more often detected signs by general practitioners. The psychotic features signal the entry in the disease's active phase. Military general practitioners report mainly persecutive theme unusual thought content and logical and conceptual disorganization. However, premorbid elements as the psychiatric history, personality disorders and the birth season are not researched enough by general practitioners despite their importance. We find also essentials factors like cannabis consumption or skull traumatism history. The main problem of theseclues is their non-specificity and that they don't have a predictive accuracy. Yet, they are good direction's elements in a medical practice in which the assessment of the job adaptation prognosis has to be made in a short and limited time
