L'observance thérapeutique : ses facteurs et ses enjeux

Abstract : A new interest has developed in therapeutic non-compliance in the past thirtyyears. It goes far beyond the simple administration of a treatment. This conceptcomplies with the rules of hygiene and dietetics and also integrates the follow-up ofdoctors' appointments as well as the ensuing tests prescribed. Its consequences,sometimes serious, on an individual or collective basis, have become a major healthproblem. In the present work, the author analyses, by means of a review of the literature, the various factors which could contribute to influencing patient compliance. He devotes the first part to the definition, assessment and consequences of non-adherence. The second part deals with a study of the factors related to the illness itself and a third part examines the factors affecting both patient and treatment. Such notions are essential to an understanding of the complexity of therapeutic compliance. The fourth part shows how effective both the doctor and doctor-patient relationships can be on adherence.The fifth part describes how much compliance can be influenced by patients'"beliefs". This approach, however, mainly focuses on the patient' s disease and doesnot allow the patient to play an active part towards a better compliance; he is notconsidered as someone responsible for his health. In the fmal part, the author develops a new approach, in which the patient is attributed an active role. He becomes a partner, sharing the responsibility of his care with the doctor. This concept is the only one which respects his point of view. Multidisciplinary teamwork is greatly emphasized and the psychiatrist plays an ever-increasing part.
