Polymédication de la personne âgée: étude des caractéristiques et déterminants

Abstract : Iatrogenesis in the elderly is a major public health problem, whose polymedication is the main factor,and is responsible of hospitalizations for 5 to 10% for patients over the age of 65: yet can be prevented. The improvement of care needs to be identified through problems arising with the treatment prescribed from the outset.Our prospective study focused on the analysis of prescribed medicine among 74 patients aged 65 or over, admitted into a geriatric day ward, during January and February 2008.The average prescribed medication at the start was 5.76, and an average of 6.23 by the end of the study. A total of 51 drugs were prescribed, for 17 interrupted. The main drugs introduced were for memory and antiagregants- taking into account the reasons for hospitalization.Interrupted drugs -mostly diuretics- showed undesirable effects such as orthostatic hypotension.What has emerged from this study is the necessity of finding a balance between the maintenance of necessary therapeutics (medication) for the patient, and the interruption of treatment with debatableefficiency and/or the potential to be dangerous. The General Practitioner plays a primary part in this exercise, because he is at the heart of the health care system, and more importantly, the connectionbetween patient and specialist.
