Le complexe démentiel du sida à l'ère des traitements antirétroviraux hautement actifs

Abstract : The author deals with the impact of highly active antiretroviral therapy inthe AIDS dementia complex (ADC).Firstly, the general aspects of infection with the human immunodeficiencyvirus are discussed, highlighting disease prevention and managemen.Secondly, a review of ADC-related literature is made together with asummary of CUITent knowledge.Finally, the characteristics of 30 patients with ADC, diagnosed betweenJanuary 1, 1988 and December 31, 2002 and followed up at the UniversityHospital Center of Nancy, are studied. The author identifies the differencesbetween patients grouped according to therapeutic strategy and illustrates her findings with five case reports.Since the large-scale use of tritherapy (second semester 1996), the numberof cases of ADC has fallen below that expected. Furthermore, for most of thepatients still alive at the end of the study, a lasting c1inical improvement wasobserved, together with a statistically significant increase in survival rate.However, the number of ADC cases with early disease has increased. Thissuggests that regular neuropsychological tests should be carried out in infectedpatients.
