L'enfant polyhandicapé et l'hôpital: étude d'une cohorte de 31 patients accueillis en 2004 à l'hôpital d'enfants de Nancy

Abstract : The child with multiple disabilities frequently has to go to the hospital throughout his life. The study carried out in the Children Hospital of Nancy in 2004, records 59 stays for 31 patients. Children are mainly addressed by special care establishments. Most hospitalizations start on friday or at the week end. Three in four stays take place in Paediatrics department, and one in five in paediatric reanimation. Hospital admissions are for a pneumological reason in 45% of cases, and for a neurological reason in 24% of cases. Hospitalizations aren't regulated in 70% of cases, and generally, the child is then addressed by a medico-social establishment. The study highlights a lack of coordination between professionnals of special care establishments and hospital staffs, and difficulties for adults hospital to take over from children hospital, when children become adults. It shows the need for time to take care of these patients. Solutions can be planned to improve the situation: creation of care networks, of which the general practitioner could be a link, call center, specifie medical record card following the child during his stay in hospital.
