Grossesse après conisation. Etude transversale des conséquences obstétricales de la conisation sur la grossesse réalisée à la Maternité Régionale Universitaire de Nancy entre 2002 et 2010

Abstract : Introduction: The standard treatment of severe cervical dysplasia is currently conization. This action induces a decrease in cervical length which can cause complications in pregnancy outcomes.Objectives: Evaluate the effect of cervical conization on pregnancy.Methods: A retrospective study was performed. The study group comprised 54 women who had a conization and that had a subsequent singleton pregnancy between 1 January 2002 and 31December 2010. Variables considered includes maternal excision date, surgery procedure, duration and week of pregnancy, mode of delivery, cone excised depth. Comparison was made with a control group of patients who delivered in our maternity (2 patients not exposed for one exposed).Results: We demonstrated a significant difference for the population exposed to the risk of premature contractions (p = <0.001) and the risk of preterm delivery (p = 0.007). We found no significant difference in the risk of pPROM [Preterm premature rupture of membranes] (p = 0.08), duration of labor, birth weight and neonatal hospitalization.Conclusion: Our study showed that previous cervical conization is a risk factor forpregnancy. The obstetrics outcomes appear to be increased significantly for large resections. It is imperative that clinicians perform the least destructive procedures whenever possible to safeguard the future of our obstetrical patients of childbearing age while respecting the oncological requirements
