Cyclo-affaiblissement transscleral au laser diode dans le traitement des glaucomes réfractaires: suivi prospectif de 35 patients sur 6 mois

Abstract : The author presents the results of a prospective study to evaluate the diode laser for treatment of 35 refractory glaucoma. The aim of this study was to assess efficacy and safety of this technique. Thirty-five eyes of thirty-five patients were included, either in a pain reducing or in IOP reducing goal, depending on initial visual acuity. Mean follow-up was 12,2 ± 5 months. Overall mean IOP showed significant lowering from mean baseline 41,1 ± 9,6 mmHg to 17,3± 8,9 mmHg at 6 months follow-up (p<1.10[exponent - 4]). Control of pain was achieved in 85% of cases, and local and general medication were significantly reduced. Loss of visual acuity was observed in 30% of the patients after 6 months, but no case of total visual loss was reported. The most frequent complication was mild iritis, always spontaneously resolutive, with anti-inflammatory treatment. Two cases of hypotony were reported at 6 months. Complications were few and mostly benign, in comparison with other techniques of cyclodestructive procedures.Contact transscleral cyclophotocoagulation with diode laser in refractory glaucoma seems to be an effective and safe technique in decreasing IOP and in controlling chronic pain.
