Conception et mise en oeuvre d’un programme d’éducation thérapeutique du patient visant la prévention des chutes après accident vasculaire cérébral

Abstract : Stroke is the leading cause of physical disability in adults in France. The statistics of Guadeloupe on stroke rank as a regional priority. The aftermath leading to falls in 46-73 % of patients during the first year post stroke. The SSR service of Choisy Clinic wanted to design and implement an ETP program on prevention of falls in post stroke suited to the population, context and organization of the service. The program design is the result of a multidisciplinary working on therecommendations. The particularities of the context and the target population required specific adaptations. The program is structured in four steps with an individual educational diagnosis, definition of the program and personalized objectives, planning of the various group workshops (six themes) and/or individual (four themes), and evaluation of acquired skills and program. The program is personalized and patient-centered. Some assessment tools have proven maladjusted; the training of professionals and integration of caregivers have to be improved.
