Biomatériaux de comblement pour la réparation de lésions ostéochondrales : intérêt des éponges polysaccharidiques à base de hyaluronate de sodium

Abstract : Hybrid Biomaterials (cell seeded materials) are currently under extensive investigations as scaffolds for osteochondral defects. In the present study, a sodium alginate solution (Alg), loaded or not with sodium hyaluronate (HA), was freeze-dried affording sponges of average porosity 174 k 98 Fm. The in vitro investigation evidenced an efficient cell seeding of the materials. The presence of HA resulted in some increase of the cell-extracellular matrix adhesion and in some enhancement of both DNA and proteoglycans synthesis. In addition, with these culture conditions, the observatian of cell cytoskeletal organization together with the detection of type II collagen demonstrated the conservation of the chondrocyte phenotype. Both types of sponges (with or without HA), as well as Alg - HA materials seeded with autologous chondrocytes were implanted in a rat osteochondral lesion model. This in vivo assessment evidenced a higher efficacy of hybrid materials for the repair of defects. At the end of the repair process, besides satisfactory histological assessments and no detection of infiammatory phenomena, the biochemical composition of neo-synthesized tissue was close that of healthy cartilage. On the other hand, the implants had no hannful effect on the cartilage of the lesion outlines. In all cases, the presence of HA tumed out to be an additional parameter favouring integration of the integration of the implant and enhancing the quality of the neo-synthesized tissue.
