Etude pharmaco-toxicologique du Doxapram chez le nouveau-né

Abstract : Doxapram, a potent respiratory stimulant, is used in the treatment of apnoea in newbom inffmts. The aim of our work was to improve doxapram Llse by studying doxapram pharmacokinetics and tolerance in neonates. A high performance liquid chromatograpny microassay with UV detection has been developed and evaluated, allowing daily therapeutic drug monitoring. Safety of administration of doxapram by intravenous infusion was ensured by demonstrating the stability of the prepared solutions. Characteristics of doxapram pharmacokinetics were studied in the newbom. Dosage, way of administration and therapeutic monitoring for enteral route were determined. Population pharmacokinetics study showed variability between the newborns. Elimination was higher in girl babies and increased with postconceptional age. Tolerance of doxapram was studied in the newborn. Analysis of retrospective data from therapeutic drug monitoring allowed to determine therapelltic and toxic plasma levels. A prospective study showed a statistically significant but moderate lengthening of QTc induced by doxapram. In conclusion, a safe use of doxapram in premature infants involves heart and plasma drug monitoring in order to reach therapeutic concentrations and to diagnose excessive levels.
