Imagerie radio-isotopique quantitative du remodelage cardiaque ischémique chez le rat : mécanisme et impact de la thérapie cellulaire

Abstract : Heart failure is generally related to an excessive and "pathological" cardiac remodelling. The aim of our study was: (i) to characterize the situations where ventricular remodelling is excessive and deleterious: evolutions of left ventricular volume and collagen fibrosis; quantification of necrosis and residual ischemia and (ii) to evaluate the impact of injection of bone marrow mesenchymal stem cells on remodelling. We developed and adapted two new quantitative techniques of imaging on the rat coronary occlusion model: [1] the Pinhole Gated-SPECT, which allows the monitoring in vivo of perfusion, extent of necrosis and left ventricular function in adult rat and [2] the [mu]IMAGER, which allows identifying the infarcted and ischemic segments for the analyses of fibrosis on histological slices. We have: (i) validated both techniques, (ii) shown that, after coronary occlusion in rat, variability of remodelling was explained by variability of the initial extent of transmural infarct. ln studies applied on cell therapy we have: (i) quantified the initial myocardial retention of transplanted cells and (ii) determined the localization of transplanted cells with regard to infarction site (dual energy [exponent 111]ln/[exponent 99]mTc-Sestamibi Pinhole SPECT). The results have show that this therapeutic intervention was associated with a reduction in ventricular remodelling and with an improvement of perfusion. This improvement was, however, mainly documented in the infarcted segments showing a significant residual perfusion before treatment (uptake of [exponent 99]mTc-Sestamibi > 50%).
