Applicabilité internationale et généralisation transculturelle d'instruments de recueil de données pour l'évaluation des systèmes de santé bucco-dentaire

Abstract : Since the end of the 70s, a series of collaborative studies conducted in the industrialized countries contributed to the development of standardized instruments for the assessment of oral health care systems across nations. To date, the application of these tools has not yet been extended to the developing countries. The present research was conducted to investigate their cross-cultural transferability in a non-westem context, Burkina Faso, West Africa. Qualitative and quantitative methods used for this purpose are described. At the system-level, the data collection form applied in Burkina Faso -for a comprehensive description of the oral health care system and socio-environmental factors influencing the populations' oral health outcomes- contributed to delineate the extent to which an instrument conceived in a western context is internationally generalizable. The role of traditional medicine and the involvement of traditional healers in the field of oral health are described. At the individual-level, the measurement equivalence (reliability, factorial structure similarity across cultures) of the questionnaires was assessed using data from New Zealand, Poland, and Germany. Focus groups discussions were conducted in Burkina Faso to assess the cross-cultural equivalence of oral health-related constructs (quality of life, behaviors, and beliefs) and the generalizability of related instruments. The promising results of this series of investigations are encouraging for an international application of the instruments. Relevancy to different socio-cultural contexts was found, but the transfer in developing countries requires cross-cultural adaptation to include new items or culture-specific module.
