Traitement prolongé par témozolomide après radiochimiothérapie dans les glioblastomes de l'adulte. Une étude rétrospective bicentrique

Abstract : Background: Glioblastomas (GBMs) are the most common primary malignant braintumors in adults. Radiation therapy (RT) with concomitant temozolomide (TMZ)followed by six cycles of adjuvant TMZ is the standard treatment after surgery. Todate, few data have assessed the impact of additional cycles of TMZ on survival.Patients and methods: We conducted a bicentric retrospective study of GBM patientsdiagnosed between 2007 and 2010. Patients considered at least with a stable diseaseafter six cycles of adjuvant TMZ received either additional cycles of TMZ or a simplemonitoring.Results: Fifty-eight patients were included. The median age and Karnofskyperformance Scale at diagnosis were 58.2 years and 80, respectively. Surgeryconsisted of resection for 48 patients (82%). All patients received RT withconcomitant TMZ. Thirty-eight patients (65%) received six cycles of adjuvant TMZand 20 patients (35%) received nine or more cycles with a median of 14 cy cles.Patients in the 6C group had a significantly higher risk of recurrence compared to thepatients in the 9C group (HR=2.25, CI95% [1.05-4.82]; p=0.03). The overall survivalwas significantly higher in the 9C group (HR=3.22, IC95% [1.30-8.00]; p=0.01) inmultivariate analysis , w ithout significant increase in toxicity.Conclusion: Prolonged administration of adjuvant TMZ after concomitantradiochemotherapy is well tolerated. There is growing data, including the presentstudy, suggesting a benefice of prolonged treatment on progression free and overallsurvival. Prospective studies in larger populations are needed, notably in order tobetter define the population to whom it can be p roposed and its optimal duration
