Les sympathectomies thoraciques et lombaires : Résultats à court et long-terme sur l'hyperhidrose et l'érythrophobie

Abstract : Background : Thoracic and lumbal sympathectomy is the reference treatment for hyperhidroses and facial blushing but with the occurrence of compensatory sweating which varies depending on the patient. This is a retrospective study on the short and long-term results of this operation. Methods : Between 1994 and 2008, 360 patients underwent sympathectomy for hyperhidroses (258 patients) or facial blushing (98 patients). Pre and per operative data were collected for all patients. A postal questionnaire was sent to the 203 patients that could be found and 155 answers were received. The mean follow-up time was 6,09 years. Results : The average age of the patients was 28,7 years. The resection levels vary with time, but the majority were resections T2-T5, no difference on side-effects were noted. Some limited resection needed to be re-operated due to a poor efficacy. 3,3 % of the interventions needed a chest drain for pneumothorax. Neurologic complications (Horner Syndrome, radial paralysis) occurred after 2,2 % of the interventions. Compensatory sweating occurred in 94 % of the patients, mainly on back (62%) and abdomen (57 %) but only 52 % had abundant sweating. Only 21 % of the patients considered to be very handicapped by this compensatory sweating. 86,7 % of the patients did not regret the intervention. Conclusion : Sympathectomy is a suitable method of treatment for hyperhidroses and facial blushing. A disabling compensatory sweating can sometimes occurs. Therefore, patients need more information about this compensatory sweating which seems inescapable.
