Risque de développer un cancer après une transplantation rénale: étude rétrospective chez 504 patients transplantés au CHU de Nancy

Abstract : To day it has been clearly established that every immunodeficient patient has a highrisk of developing a tumour. A malignant pathology is a frequent complication in a renal transplant although present epidemiological data is limited.A forward study of 504 c1inical files of kidney transplants was monted over a period of eleven years from the 1st January 1985 until the 31 st of December 1995, the cancer was first diagnosed, the commencement date of dialysis and updated finding up to the 31st December 1999.This population sample can be compared to the general population in France overthe same period of time thanks to the FRANCIM network.Within the transplant population, 87 patients developed a cancer, of wich 45 werenon-cutaneous and 42 cutaneous. This study shows evidence of a high risk of various cancer. This high risk group only concerns: Cancer of the prostate gland, kidneys and lymphoma in men and cervical cancer, kidneys and Iymphoma in women. Despite the absence of a skin cancer register in France excludingmelanomas, evidence indicating a high incidence of skin cancers with apredominance of spinocellular epitheliomas has been noted.The elevated risk rate of prostate cancer, kidney and cervical cancer may be linked to systematic screening procedures. On the other hand there is no increased risk pertaining to cancers frequently observed within the general population in France such as breast cancer, colorectal and lung cancer.
