Comparaison de deux méthodes d'évaluation de la ration calcique chez des femmes ménopausées consultant en médecine générale et intérêt d'une supplémentation en calcium dans la prise en charge de l'ostéoporose

Abstract : Calcic supplement among post-menopausal women enables to slow down bonemass reduction and to dicrease the risk of fractures according to some authors and under certain conditions. This strategy valuation study will enable a doctor to quickly determine the existence of calcium deficiency in his/her post-menopausal patients. This study conducted here compared two valuation methods of the calcium requirements among 683 post-menopausal women. The CoCoNut-Calcium score is a frequency questionnaire of calcium weekly consumption. These results were compared to the answer to the question "Do you consume a calcic product at each meal?". This test has a very positive predictive value of 95.9%. This enables us to make a pre-selection. Women who answer no to the question "Do you consume a calcic product at each meal?" and who therefore do not consume 3 dairy products have a deficiency in calcium. Women who answer yes to the samequestion must be subject to further study in order to detect a possible deficiency. French recommendations are 1200mg calcium per day for post-menopausal women and are probably a little excessive. They have for objectives to protect 100% of the population. Calcium intakes that seem reasonable and high enough to protect 80% of the population are more around 800 to 1000mg of calcium, which by the way matches the consumption of 3 dairy products. Advising the consumption of 3 dairy products per day is therefore a health public message allowing to fight against osteoporosis in post-menopausal women. Food advicesgiven by the doctor are important and effective provided that they are repeated along numerous consultations and that we insure they are well-implemented. The message can be taken over by a dietician, which will increase even more the modification chances of eating habits
