Réinterventions pour rupture itérative de coiffe des rotateurs : à propos d'une série de 50 cas à un recul moyen de 7,1 ans

Abstract : INTRODUCTION : Literature regarding the outcomes of revision rotator cuff repair is limited. The impact of re-tear may reach 90% and usually varies between 25% and 35%. The purposes of the present study were to report the tendon repair integrity and clinical outcomes for a cohort of patients following revision rotator cuff repair and to examine factors related to tendon healing and the influence of healing on clinical outcomes. METHODS : Fifty patients with revision rotator cuff repairs were retrospectively reviewed with a mean postoperative follow-up of 7.1 years. Outcomes were evaluated on the basis of a visual analog pain scale, the Simple Shoulder Test and the Constant score. Ultrasonography, CT Scan and MRI were used to examine repair integrity following surgery. RESULTS : The average age of patients was 60.5 years (45-93), the period between the two interventions was 3.6 years (0.5 to 11.1). 68% of patients retain residual pain. The iterative rupture rate was 69%. The Constant score was significantly higher in patients with an healed tendon (p = 0.05). The surgical technique, fatty infiltration and the preoperative tear size did not influence clinical outcomes. CONCLUSION : Results in revision rotator cuff repair are related topostoperative tendon integrity. The healing rate remains disappointing. The surgical technique did not influence functional outcome
