Tomoscintigraphie de perfusion cérébrale dans l'épilepsie lobaire temporale pharmacorésistante : comparaison des différentes méthodes d'analyse

Abstract : Brain perfusion SPECT allows ictal cerebral blood flow increase into the epileptic focus (hyperperfusion) to be localised Comparative visual analysis (Visual - FBP) might be enhanced by subtraction SPECT Coregistered to MRI (SISCOM - FBP) and by iterative reconstruction methods that improve spatial resolution (Visual - OSEM). Goal : Evaluate accuracy in determining the involved temporal lobe (right or left) and the inter-observer reproducibility for precise localization in precisely locating of temporal and extra-temporal foci, by comparing 3 image processing methods in patients with a definite temporal lobe epilepsy. Results : A high accuracy was documented for both Visual- FBP and SISCOM - FBP (between 80 and 87 %). This accuracy was lower for Visual - OSEM (about 70 %). The total number of detected foci was greater with Visual- FBP (n = 54) and SISCOM - FBP (n = 58) than Visual- OSEM (n = 30). Inter-observer reproducibility was greater with Visual - FBP (52 %), when compared with SISCOM - FBP (43 %) and especially with Visual- OSEM (23%). Conclusion : Conventional reconstruction methods, with or without SISCOM technique, seem to be the more accurate and reproducible ones. Iterative reconstruction methods should be optimized before clinical use.
