Evaluation des pratiques professionnelles en matière de prééclampsie dans l'Ouest guyanais: Etude rétrospective de 88 cas survenus en 2007

Abstract : In a South American context where pregnancy hypertensive disorders are the first cause of maternal death, perinatality is considered in French Guiana as a public health priority. The west of French Guiana is concernedwith a high birth rate and a lack of medical practitioners. Under such conditions, improvement of obstetric care needs locally adapted solutions which can be provided by an assessment of medical practice.We led a retrospective audit of 88 cases of preeclampsia registered in 2007 at the obstetric care unit of Saint Laurent du Maroni's hospital. We followed the methological recommendations provided by the French HauteAutorité de Santé and compared the local medical practice to the French and international recommendations about preeclampsia.In our study, the incidence of preeclampsia was of 4, 1 % given that preeclampsia is not weIl registered in thishospital. However, the actual incidence in the west of French Guiana should not be very much higher and is certainly in the common range of 2 to 7% of pregnancies. On the other hand, the incidence of eclampsia wasvery high (4,2 %0). In the literature, a high rate of eclampsia is usually associated with poor essential obstetriccare. After studying 15 criteria, we notice that obstetric care quality about preeclampsia can locally be improved by :- a better collaboration between primary care practitioners and specialists concerning follow-up of women athigh risk for preeclampsia and in the postpartum period,- limiting the use ofthe uterine artery doppler to women with high risk for vascular placental diseases,- a greater implication of anaesthetists in the management of severe preeclampsia,- a better information system for every pregnant women about symptoms preceding the eclamptic ceisure.
