Résultats de la prise en charge des traumatismes sonores aigus : à propos de 225 patients

Abstract : Acute acoustic trauma is frequent in the armies with an incidence about 3/1000 a year. The internal ear's lesions caused by an intense impulse noise can give hearing loss and tinnitus. The place of military practitioner is essential in the prevention, the treatment and the follow-up of these patients. Our study concerns 225 patients hospitalized for acute acoustic trauma between 2003 and 2008. It leans on the analysis of audiograms in the entrance and in the exit of the hospitalization and that in D+30. Tinnitus and hearing loss are the most frequent symptoms. The hearing loss averages initial is 33,982 dB with as target frequencies 4000 and 6000 Hz. We observe a recovery after treatment of 18,278 dB distributed in 13.846 dB during the hospitalization and 4,432 dB between the exit and D+30. It seems that the threshold of 40 dB of initial average hearing loss is a threshold from which the rate of audiometric aftereffects and the obstinacy of acouphenes increase in a significant way. 77 % of patients were taken care medically before the twelfth hour underlining the central role of the military practitioner
