La réhabilitation vocale selon Pearson. A propos de 49 cas. Evolution et prespectives de la chirurgie carcinologique pharyngo-laryngée

Abstract : This work mainly consisted in rehabilitating 49 patients, following Pearson's near total laryngectomy procedure. That surgical act was described 20 years ago. It is reported with indications and results from literature. This work was decided withthe G.E.T.T.E.C. : le "Groupe d'Etude des Tumeurs de laTête Et du Cou".The last part describes perspectives of laryngeal rebuilding astheyappear in literature. Voice rehabilitation according to Pearson consists in creating a tissue tracheo-Iaryngopharyngeal shunt, sothatthe patient can speak with hisown lung air capacity. This shunt is created afternearly totallaryngectomy, due to cancer.Indications are the same as in case of pharyngo-Iaryngectomies if structures can bepreserved so asto build theshunt. Survival after3 years is similar to total (pharyngo-)Iaryngectomy. Functional results are satisfactory butmay showstenosis and lor swallowing impaires. Near-total (pharyngo-)Iaryngectomy according to Pearson appears as an improved alternative to traditional total (pharyngo-)Iaryngectomy. Still, Pearson's procedure cannot bean alternative process to chimio-radiotherapy. Butit allows correction in case of failure. Surgical restoration of laryngeal functions was extensively studied in 1965-85. But it mainly consisted in restoring phonation and continence against food inhalations. Suppressing tracheostoma is much more daring and astate of the art technique in order to restore physiological breathing.
