Les néoplasies intraépithéliales vulvaires : à propos dune série de cinquante et une patientes

Abstract : Objective: Vulvar intraepithelial neoplasia (VIN) are the main precursors ofsquamous cell carcinoma of the vulva. Diagnosis and appropriate management ofVIN is a real medical issue. This study aimed to evaluate treatment modalities andrisk factors for recurrent VIN.Methods: Fifty-one patients with usual (grade 2 or 3) or differentiated VIN wereincluded in retrospective multicentric study conducted at the Centre Alexis Vautrinand Regional University A. Nancy Pinard Maternity between January 1995 andMarch 2010.Results: Usual VIN is a pathology of young women, HPV-induced, multifocal andmulticentric with a low malignant potential. In our study, usual VIN preferentially aretreated by laser vaporization in order to minimize anatomical injury. Imiquimod andphotodynamic therapy, are other possible treatments for this condition. Thedifferentiated VIN is often associated with vulvar lichen sclerosus. The treatment issurgical due to a risk of progression. Risk of recurrence was significantly higher inCO2 laser group without difference in progression to squamous cell carcinoma.Conclusion: Our results are consistent with literature data. They recall risk of VINrecurrence and interest to regular monitoring, close and extended
