Etude de la prise en charge des toxicomanes en médecine de ville: enquête de pratiques professionnelles auprès des praticiens généralistes et des pharmaciens du secteur sanitaire de Saint-Nicolas-de-Port (Meurthe-et-Moselle)

Abstract : The care of drug addict patients has known a continuous evolution due to the development of the drug addiction phenomenon and scientific progress. The urban general practice was affected by this in the 1980's withthe policy of risk reduction just after the beginning of the AIDS epidemic. At that time, general medicalpractitioners and chemists had very little experience of drugs and looking after drug addicts.We have carried out a survey of the opinion and practice of general practitioners and chemists workingin the Saint-Nicolas-de-Portr ea. This survey was aimed at assessing how drug addicts are looked after andsuipported, identifying assets and difficulties as well as training needs of health professionals. It appears that urban general practitioners have various assets and means at their disposai : frequentcontacts, knowledge of patients and their social environment, substitute treatment. However, time investment, inappropriate training and above all the complexity of drug addiction. a multifactorial pathology, are strongcurbs which limit practitioners involvement and make the care of drug addict patients in towns very uncertain : some of the general practitioners refuse to treat such patients, while otliers overinvest in this activity.It seems obvious that, to be done in good conditions, the care of drug addict patients requires the development of relations with several partners. It must be based on a multidisciplinary approach, that exists onlywithin local networks. Such networks allow the care of drug addict patients to be complete.
