Etude de l'intérêt du MorphoTEP au F-FDG dans la prise en charge initiale des cancers du col utérin

Abstract : FDG-PET imaging is not rontinely used in cervical cancer staging. After a brief recall on the main topics of FDG-PET and cervical tumors, a retrospective stucdy on patients for whom a FDG-PET had been performed as part of the work-up of the cervical cancer at initial diagnosis, has been presented. The aim of this thesis was to retrospectively assess the accuracy of FDG-PET-CT in the initial staging and to determine its impact on the therapeutic strategy. For this propose 85 patient had been selected over a txo year pariod. FDG-PET acquisitions were performed 60 min after IV injection of 5.5 Mbq/kg 18F-FDG using a Biograph [Registered trademark] (Siemens) PET-CT imager. In order to evaluate diagnosis performances of this technique, PET-CT data were compared to those issued from conventional imaging (CI) and correlated to histology and/or further evolution. How the patient management was affected by PET was also a question we addressed. The results of this study can be summarised as follows : 45 patients met theinclusion criteria. PET correctly identified 13 out of 16 metastatic patients. Discrepancies between PET and CI in 11 cases out of 45 have resulted in modifications of the therapeutic strategy for 8 patients. To conclude PET-CT brings further valuable data to CI, implying changes of the treatmenlt strategy. These results have to be confirmed by prospective studies including a more important population.
