Evaluation structurale et inflammatoire (épanchement et synovite) par échographie dans la gonarthrose : comparaison aux données cliniques, radiographiques et IRM

Abstract : In osteoarthritis (OA), X-rays are usually considered as the gold standard in clinical practice but they are a lot less sensitiveless sensitive than MRI in detecting early cartilage lesions and abarticular abnormalities. However, MRI is timeconsumjhg and expensive. Here, we studied for the first time the performances of an ultrasound (US) examination of knee OA: cartilage lesions in femoral condyles,, joint effusion and synovial membrane inflammation were assessed with this technique. US data from 23 patients suffering from knee OA werecompared to clinical and imaging data (X-rays and MRI). Sixteen patients had an OA diagnosis according to Xrays, 20 according to US and 23 according to MRI. No correlation was found between cartilage damages, joint effusion and synovitis and pain or disability (for all imaging technique). Correlations between MRI and US datawere rhoderate for cartilage damages (r=0.43; p<0.05) and joint effusion (r=0.73; p<0.0001), and good for synovial inflammation (r=0.75p; <0.0001). In comparison, correlations between structural damages seen in Xraysand MRI were moderate to good for medial and lateral tibio-femoral compartment : r=0.39 and r=0. 68 (p<0.05) respectively. In conclusion, in knee OA, US is a valuable imaging technique and could be advantageously added to X-ray evaluation to assess cartilage and abarticular lesions (joint effusion and synovial inflammation).
