Apport de la chirurgie dans le traitement des tendinopathies calcanéennes chroniques : à propos de trente six cas

Abstract : This study examines the cases of 36 patients who received surgical combing for a chronic calcaneal tendinopathy that proved resistant to medical treatment. Anatomical and physical properties of the tendon are discussed in the first part of the study. Next, the different types of tendinopathy, as well as diagnostic differentials, are described. Finally, the principals behind a conservative treatment and the surgical techniques and their indications are explained. A description of the series and the method of evaluation precede the findings of the study. Theanalysis of these results supports the recommendation of surgical intervention in the treatment of chronic calcaneal tendinopathy, when the indications are respected, with 86.1 percent displaying good to very good results. It seems important to try and define what qualifies a patient as being "good" for this type of surgery. In the series under examination, a good prognosis regroups the factors of sex, age, level of physical activity and the type of tendinopathy, without a statistic or scientific link having formally been established. Calcaneal tendinopathy surgery is known for providing positive results over the short term. Our study,conducted over a 12-year period, provides evidence for the stability of these results over time. A comparison with other studies published in the literature leads us to think that even better results could be obtained by optimizing the duration of immobilization and the post-op follow-up, in particular at the level of functional reeducation. The interest of a multidisciplinary account seems evident. The emergence of new, less-intrusive therapeutic techniques, which still need to be evaluated, is underscored in the end
