La dermatologie en médecine générale : rôles et enjeux de la correspondance médicale exemple de l'hôpital d'instruction des armées Legouest

Abstract : During consultations in dermatology at the HIA Legouest, we find variability of content and presentation of letters from general practitioners. Difficult task of drafting, however, we wonder if it is not possible to build a standard response to facilitate their daily practice. Our task was to redefine the place of mail as a critical tool for the management of the patient in the new term care. Then we received letters from general practitioners to disclose the information present or not and to measure the expectations of dermatologists. The comparison of 445 letters from doctors with the advice of 45 dermatologists helped identify areas for improvement : the medical criteria prior to the consultation are not present indicating the usual treatment in 20% of known cases, background in 28.33% of the patient, lifestyle (alcohol and tobacco) in 2.28% and 2.92% indicated in allergies. The administrative criteria are well informed and in a lesser extent the medical criteria on the basis of the consultation. The computerized letters in this study are better informed than the manuscripts. A discussion with practitioners on drafting modalities in practice showed that the working conditions (time, number of visits, etc..) Since the introduction of care pathways largely conditioned the development of mail. We made an example of sample letter and highlighted the contribution of it support as a key element of medical correspondence tomorrow.
