Intérêt de l'arthrorise avec remodelage du second métatarsien avec le cunéiforme intermédiaire dans les luxations tarso-métatarsiennes : à propos d'une série rétrospective observationnelle de 15 patients

Abstract : Introduction: dislocations of the tarsometatarsal joint trauma is a rare lesion entity. We believe that one of the causes of failure is associated with reduced and inadequate stabilization of second metatarsal in its anatomical position , therefore causing an imbalance in the metatarsal keyboard can influence the long-term functional results .Patients & Method : The main objective of this study was to evaluate the interest of the arthrodesis with remodeling of the base of the second ray with the medial cuneiform in the management of dislocations tarsometatarsal joints of type B2 and C according Myerson classification . Between 2007 and 2009 , 19 patients were supported surgically in our center for a tarsometatarsal dislocation type B2 or C.Results: 15 patients have been included in the study, we identified 9 patients who received arthrodesis specifically with remodeling of the joint of the second metatarsal with the intermediate cuneiform . The average age was 42.3 years. 8 patients have been supported in the context of an accident. 9 patients were workers of the secondary sector . The mean time to hardware removal was 3.6 months .Discussion : No statistical difference was demonstrated with regard to the main objective. The only statistically significant change and value is the distance MT 1-2 seems to be better corrected in the group who underwent arthrodesis with remodeling of the second ray (p = 0.0413 Wilcoxon test ) but no detectable clinical correlation in terms statistics ( p = 0.4432 AOFAS Wilcoxon test ) despite the influence of this value on the risk of instability. We observed a large number of complications in this series.Conclusion: arthrodesis with remodeling of the second ray does not seem to be, statistically significantly better clinical outcome. The surgical strategy should include a reduction in the open of the three columns representing the Lisfranc joint and stable fixation with equipment complying with the articular surfaces
