Efficacité du cétuximab en situation néo-adjuvante dans les traitement des carcinomes épidermoïdes cutanés localement avancés inopérables

Abstract : Efficacy of neo-adjuvant cetuximab for the treatment of unresectable locally advanced cutaneous squamous cell carcinoma (cSCC).OBJECTIVE: To evaluate the efficacy of induction treatment with cetuximab (an epidermal growth factor receptor inhibitor) in making locally advanced cSCC resectable.METHODS: A retrospective study at Gustave Roussy Institut, from February 2008 to March 2013, including the patients with an unresectable localy advanced cSCC treated with neo-adjuvant cetuximab combined with platinium salt and 5-FU (CC) or in monotherapy (MC) RESULTS: Thirty-seven patients were evaluated (median age 72 years, n=28 CC; n=9 MC). After 3 cycles, 31 patients became resectables (83.8%) and 24/30 operated patients (80%) had a complete resection. Progression free survival and overall survival in operated patients were respectively 9.1 and 24.7 months. Tolerance was acceptable.CONCLUSION:This study shows good response for resectability and tumor control, with few relapses despite the poor prognosis of these carcinoma initially and the elderly population group. However, this therapeutic schedules needs to be confirmed by studies of higher level of evidence
