Plaie chirurgicale du talon chez le diabétique: quels sont les facteurs pronostiques

Abstract : A foot wound for a diabetic always represents a senous incident which can end up in amputation. The heel is not the most common location but it often develops in a nasty way. Conservative surgery has made some progress and permits in some cas es to avoid amputation. It can consist in a plain debridement , in a skin graft, a flap or a calcanectomy. By studying 43 cases of diabetics suffering from heel surgical wound, we intended to estimate the part of this surgery. We also wanted to check if there were prognosis factors for permitting to choose thiskind of treatment for patients. In our study, nearly 60 % of the subjects could benefit from this surgery. A statistically significant increase of the risk of amputation can be noticed if the patient has an antecedent of wound, if he shows infectious signs on the blood balance or if the ulcer is located on the left heel.We have no explanation as far as this last result is concerned.
