Suivi en IRM des tumeurs desmoïdes : corrélations radio-cliniques

Abstract : Purpose. To evaluate the impact of hormonal therapy on MRI characteristics ofdesmoid tumors on T1 weighted, T2 weighted fat-saturated and post-contrastsequences.Materials and Methods. 19 patients with histologically-proven desmoid tumorswere prospectively followed up on MR imaging. Eight patients underwenthormonal therapy and 11 were surveyed. Change in tumor size during follow-upwas analyzed according to RECIST criteria. Signal intensity on T1 weighted, T2weighted fat-saturated and T1 weighted fat-saturated post-contrast images wasgraded from 0 to 5 using adjacent normal muscle as reference. Findings werecorrelated with tumor growth and treatment option.Results. 26 follow-up periods were evaluated: eight of tumor progression and 18of tumor stability/regression. Hormonal therapy was associated with tumorstability or regression (p = 0.0207). There was a significant reduction inenhancement among treated patients with stable/regressing disease (p = 0.049).The mean variation in enhancement grade was -1.3 ± 1.2 in these patients. Allsuccessfully treated patients presented a reduction in enhancement. Marked lowenhancement or T2 signal were rare in progressing lesions (0% and 13%).Conclusion. Hormonal therapy has an impact on desmoid tumor signalcharacteristics. Reduction in enhancement is associated with a positive responseto hormonal therapy
