Comparative study of 3T and 1.5T MRI with T2FSE and diffusion (DWI) sequences for the detection and characterization of focal liver lesions, using histology as a gold standard.

Abstract : Purpose: To prospectively determine the performance of detection and characterization of focal liver lesions (FLL) at 3T MRI versus 1.5T MRI, using T2FSE and DWI sequences, compared to the gold standard of histology.Materials and methods: We carry out a prospective, single center, uncontrolled,unrandomized, open study on 40 consecutive patients. 2 MRIs were performed the same day, 1 to 31 days before a liver surgery, systematically associated with an intraoperative US. We first performed a 3T MRI (DWI, high resolution FSET2 : FSET2HR, standard FSET2: FSET2S), followed by a 1.5T MRI (DWI, FSET2 and 3DT1 dynamic contrast-enhanced imaging). The isolated and joint DWI and FSET2 sequences were studied (randomized, blind) by 2 radiologists. Finally, we compared the MRI data (artifacts, detection and characterization) to the gold standard, histology for the removed fragment and to the optimal count (complete imaging study and post-operative follow-up) for the remaining liver.Results: 119 lesions were found including 104 on surgical specimens): 77 metastases, 10 HCC, 1 intrahepatic cholangiocarcinoma and 28 of other kinds. Except an aberrant value for DWI-3T for one observer, sensitivities of detection varied from 77.31% for FSET2-1.5T to 84.87% for FSET2HR-3T, insignificantly improved by joint analysis of sequences (FSET2+DWI): 89.08% at 3T and 82.35% at 1,5T. The rate of total concordance for characterization (benign versus malignant) was: 95.28% at 3T versus 94.90% in 1.5T.Conclusion: Sensitivity of detection and characterisation of FLL (using DWI and T2FSE sequences) are not statistically improved at MRI 3T versus 1.5T. DWI has no advantages over FSET2 in detection of FLL, at 3T as well as at 1.5T.
