Inhibition non médicamenteuse de la lactation : qu'en est-il au retour à domicile ?

Inhibitors of lactation are being reconsidered since the ANSM (French National Agency for Medicines and Health Products Safety) has ruled the use of bromocriptine unfavorable within the famework of ablactation. Alternative drug-free methods should be preferred. Our study was conducted on 120 (female) patients for a period of 5 months. The evaluation of symptoms and pain was done at the maternity hospital and at home with the help of two questionnaires filled out on days 3 and 21 after birth. Nearly half of the patients got anexplanation on a possible lacteal secretion. In most cases, these were released after birth despite a choice for artificial feeding before or during pregnancy. Also, the nondrug care was globally incomplete: the different alternatives were not thoroughly presented to all patients. They were numerous to feel deprived when coping with symptoms of mammary congestion, which usuallybegan when they returned home. However, the related pain, which was greater than that suffered at the maternity hospital, did not hinder their management. A drug-free care of ablactation prior to birth is mandatory because of strong evidence, and should be continued after birth in the maternity hospital and subsequently at home by a certified nurse midwife
Les inhibiteurs de la lactation sont remis en cause depuis que l'ANSM a jugé défavorable l'utilisation de la bromocriptine dans le cadre de l'ablactation. Les méthodes alternatives non médicamenteuses sont à préférer. Notre étude a été menée sur 120 patientes durant 5 mois. L'évaluation des symptômes, des douleurs et de leur gestion s'est faite à la maternité et à domicile grâce à deux questionnaires distribués à J3 et J21. Près de la moitié des patientes ont reçu des explications sur une possible sécrétion lactée. Ces dernières ont majoritairement été délivrées après l'accouchement malgré le choix d'un allaitement artificiel avant ou au cours de la grossesse. Aussi, la prise en charge non médicamenteuse est globalement incomplète : les différentes alternatives ne sont pas présentées de manière exhaustive à toutes les patientes. Elles sont nombreuses à s'être senties démunies face aux symptômes d'une congestion mammaire débutant principalement après leur retour à domicile. Cependant, la douleur, plus importante qu'à la maternité, n'a pas empêché leur gestion. La prise en charge non médicamenteuse de l'ablactation doit initialement être anténatale grâce à des explications adaptées, et poursuivie durant le séjour à la maternité ainsi qu'à domicile par unesage-femme libérale.
