Le Binge Eating Disorder en 2017 : de l’impulsivité à la compulsivité, les ouvertures du DSM-5

Abstract : The DSM-5 (Diagnostic and Statistical Manual of Mental Disorders - Fifth Edition) recognized Binge eating disorder (BED) as a distinct eating disorder. The objective of this review is to describe the diagnostic and severity criteria for BED, and to analyze the potential links with addiction. A sense of loss of control over eating and marked distress have been shown to be a core feature of BED. We propose to identify individuals with subthreshold BED, defined by a lower binge eating frequency or by a smaller amount of food consumed during the binge episode (subjective binge), because they exhibit eating disorder patterns (including loss of control and distress) and psychiatric symptoms, similar to those of subjects with full-BED syndrome. This subthreshold BED can represent a phase of an eating disorder in individuals who fulfilled criteria for BED at other times. Although it could be also a less severe disorder in terms of presentation and treatment outcomes, empirical evidence supports the legitimacy of a continuum from subthreshold BED to severe BED. In both cases, emotional eating, defined as eating or overeating in response to negative emotions, is the first symptoms. Traits of impulsivity or compulsivity and altered reward sensitivity are reported both in BED and in addictions. Neuroimaging studies suggest there are corticostriatal circuitry alterations in BED similar to those observed in substance abuse. So, in our opinion BED is a useful and valid phenotype, distinct from obesity that could be considered in the near future as a behavioral addiction.
