Axial clinical and imaging characteristics of patients with inflammatory bowel disease : a retrospective study in 51 patients

Résumé

OBJECTIF. The objective was to characterize axial musculoskeletal features and inflammatory and structural sacroiliitis in a population of inflammatory bowel disease (IBD) patients. METHODS. 51 patients suffering from IBD were followed in a gastroenterology department, 41 for Crohn's disease (80.3%) and 10 for ulcerative colitis (19.6%). Clinico-biological, endoscopic and imaging data were collected during patient follow-up. A questionary inspired from ASAS criteria permitted to obtained information about musculoskeletal complaints. Structural and inflammatory sacroiliitis was assessed on Computed tomography (CT) (TAP or abdominal) and Magnetic resonance enterography (MRE)exams by radiologist and rheumatologist. RESULTS. In this population (n=51), 27 patients (52.9%) suffered from low back pain (LBP) and 10 (19.6%) with inflammatory component. Eight of them were referred to a rheumatologist. On the 51 patients, structural sacroiliitis on CT and inflammatory sacroiliitis on MR were depicted in 4 (7,8 %) and 7(13,7%) patients respectively and on both imaging for only one patient. For patients with sacroiliitis, 6 (11,8%) had no axial complaints and 6(11,8%) patients suffered from LBP (mechanical n=5 and inflammatory n= 1) responding to ASAS criteria (LBP + inflammatory and/ or structural sacroiliitis). No axial musculoskeletal complaints or imaging abnormality were associated to any IBD features.CONCLUSION. In this population of 51 patients, 6/27 patients with LBP (mechanical (n=5) inflammatory (n=1)) presented sacroiliitis (structural and or inflammatory) and responded to ASAS criteria and 6/24 patients without LBP presented 6 sacroiliitis. This study outlines the discordance between axial symptoms and imaging abnormalities of SIJ.
