Maladie d'Alzheimer : la réticence des aidants conjoints à solliciter une aide extérieure

Abstract : The burnout of the caregivers of patients suffering Alzheimer disease - and particularly amongst them the spouses - shall not to be demonstrated anymore and the Alzheimer's plan 2008-2012 finally confirmed the recent attention granted to the caregivers. However, if the assistance to the caregivers is shown as a necessity, the demand remains below the estimated needs. Thirty-one assisting spouses whose partners were still at home - for twenty of them - or already placed in an institution - for the other eleven - have been met. An open questions directive interview was conducted. A content analysis allowed us to reveal certain position that affect such resistance to external services: it is even more stronger than spouses consider that the marital bond generates the duty of assistance, that they position themselves as guardians of the patient's identity, that they have the feeling to meet social and familial expectations and that the accompaniment bring them the sense of significant accomplishment. In addition, a gender difference of the caregiver was observed: women are significantly more reluctant than men to have recourse to assistance services. These results clearly show a phenomenon, already mentioned by a couple of workers and relayed by professionals, but still very few studied in France: the spouses caregivers are reluctant when it comes to request an external help. Beyond, they lead us to reconsider the assistance granted to caregivers by re-assigning its place to each of the triad caregiver-aided-professional and by changing our perception of the disease, as well as on this phenomenon of reluctance which, eventually, may be a necessary step in accompanying the ailing spouse.
