Etude volumique en IRM des synovites et des ténosynovites des poignets chez 25 patients atteints de polyarthrite rhumatoïde: rôle prédictif de l'évolution structurale à 1 an

Abstract : To evaluate for the first time on magnetic resonance imaging (MRI) the synovitis and tenosynovitis volume of the wrist and its enhancement speed in Rheumatoid Arthritis (RA) and to determine its predictive role on radiographic erosive progression at 1 year. Twenty-five RA patients were assessed clinically, biologically and radiographically at baseline and at 1 year.synovitis volume was calculated in wrists on 5 consecutives post-injected T1-weighted images. According to enhancement speed, 3 thresholds of synovitis and tenosynovitis were defined : high (>= 1% S[exponent -1]), intermediate (>=0.3% S[exponent -1] and <1 % S[exponent -1]) and slow (<0.3% S[exponent -1]), and their respectivevolume calculated. Structural progression is defined by the worsening of more than 5 points on Sharp's score at 1 year (SDD=4.9). Synovitis is observed in 23 patients (92%). lntracarpian joints were the most frequently involved with synovitis. Tenosynovitis was recorded in 21 patients (84%). The most common sites for tendon involvement were extensors (95.2%). The mediansynovitis and tenosynovitis volume was 837.5 mm3. Inter-observer and intra-observer reliability was high (ICC=0.93-0.99; ICC=0.75-0.95). There is no correlation between the different volumes of synovitis and tenosynovitis and the clinical, biological and clinico-biological parameters. Only CRP correlate with x-ray progression (r=0.56; p=0.003), while synovitis and tenosynovitisvolumes (total and for each category of enhancement) were not correlated.
