Cystite hémorragique après allogreffe de cellules souches hématopoïétiques: caractéristiques cliniques, facteurs de risque, traitement et évolution

Abstract : Hemorrhagic cystitis (HC) is a common complication after hematopoietic stem cell transplantation. We conduct a retrospective study among 435 patients transplanted between 1990 to 2006 to investigate clinical characteristics, treatment, outcome and risk factors for HC. This complication develops in 75 patients (17,2 %). CH begins a median of 24 days (range, 0-780) after transplantation and last a median of 30 days (range, 2-180). Significant risk factors for HC by univariate analysis include high risk disease (p=0,04), use of cyclophosphamide and busulfan (p<0,001), use of cyclophosphamide (p=0,04), use of busulfan (p=0,02) and chronic GVHD (p<0,001). Multivariate analysis doesn t show any risk factor. 48 patients with HC have polyoma BK virus viruria (culture or PCR). The children (p=0,04), the recipients of graft with a high risk disease (p=0,03) and the patients who receive an unrelated transplantation (p=0,03) have significantively more late-onset HC, which occur beyond 30 days after transplantation than early-onset HC. Use of cyclophosphamide and busulfan give more early-onset HC than late-onset HC (p=0,04). Treatments of HC are numerous. 13 patients are treated with infusions of FXIII concentrate. Patients without HC have not significantly higher overall 17-years survival than patients with HC (p=0,25).
